Forms and Supplies

IFPTE is putting its forms online. No more waiting to receive a form through the mail or by facsimile. Members only need to access this page and print the required form(s). All forms require the original signature of a Local union officer(s) and/or of the member. While we have the technology to e-mail completed forms to the appropriate individual, the request requires signatures. IFPTE will only accept signed forms. Locals and members have the option of faxing or mailing completed forms to the Federation.
